ABSTRACT

Objetivos: Identificar os fatores de risco pré e pós-operatórios relacionados ao desenvolvimento de mediastinite entre pacientes submetidos à cirurgia cardíaca em um hospital da cidade do Rio de Janeiro, caracterizar a população estudada e analisar a relação entre os fatores de risco e a incidência de mediastinite nos pacientes submetidos à cirurgia cardíaca. Método: Estudo descritivo, abordagem quantitativa, em que foram identificados os fatores de risco para mediastinite em pacientes que realizaram cirurgia cardíaca em um hospital do Rio de Janeiro. Resultados: Obteve-se um n de 192 pacientes, de maioria homens, na faixa etária de 50 a 69 anos e em sobrepeso. Diabetes mellitus e tabagismo foram as comorbidades mais frequentes, e CRVMC a cirurgia mais realizada, 4 pacientes apresentaram mediastinite. Conclusão: A identificação destes fatores contribui para elaboração de estratégias de prevenção para mediastinite, e na implementação de cuidados de enfermagem no pré e pós-operatório de cirurgias cardíacas


Objectives: To identify pre and postoperative risk factors related to the development of mediastinitis among patients submitted to cardiac surgery at a hospital in the city of Rio de Janeiro, to characterize the study population and to analyze the relationship between risk factors and the incidence of mediastinitis in patients undergoing cardiac surgery. Method: descriptive study, quantitative approach, in which the risk factors for mediastinitis were identified in patients who underwent cardiac surgery in a hospital in Rio de Janeiro. Results: a n of 192 patients, mostly males, aged 50 to 69 years and overweight were obtained. Diabetes mellitus and smoking were the most frequent comorbidities, and CRVMC the most performed surgery, 4 patients had mediastinitis. Conclusion: the identification of these factors contributes to the elaboration of prevention strategies for mediastinitis, and the implementation of nursing care in the pre and postoperative period of cardiac surgeries

Resultados: Se obtuvo un n de 192 pacientes, de mayoría hombres, en el grupo de edad de 50 a 69 años y en sobrepeso. La diabetes mellitus y el tabaquismo fueron las comorbilidades más frecuentes, y CRVMC la cirugía más realizada, 4 pacientes presentaron mediastinitis. Introducción: El estudio de las infecciones en la historia de la Medicina data de 1546, y se extiende su investigación hasta nuestros días. El abordaje quirúrgico del Sistema nervioso y la estadía hospitalaria subsecuente hace de esta entidad una amenaza constante para el éxito de la cirugía. Objetivo: Describir el comportamiento de las Infecciones en el Serviciode Neurocirugía del Hospital Universitario Dr. Miguel Enríquez. Material y Métodos: Se realizóun estudio prospectivo de 108 casos ingresados en el Serviciode Neurocirugía del Hospital Universitario Dr. Miguel Enríquezque tuvieron un cuadro infeccioso durante el período marzo 2013-agosto 2015. El universo estuvo constituido por todos los pacientes mayores de 18 años de ambos sexos ingresados en el servicio por diferentes rutas desde el cuerpo de guardia, unidades de terapia intermedia e intensivao ingresado directamente en sala. Se recopilóla información en una base de datos y se recogió el número de historia clínica, edad, sexo, diagnóstico, tipo de infección, estudio microbiológico indicado y germen aislado. Se construyeron tablas de frecuencias. Resultados: En esta serie hubo un predominio del sexo masculino (66.7 por ciento) en relación con el sexo femenino (33.3 por ciento); el grupo de edad de mayor riesgo fue entre la cuarta y sexta décadasde vida (75 por ciento); los gérmenesmás aislados fueron el Staphylococcus aureus, (18.5 por ciento), la Klebsiella (7.4 por ciento) y la Echerichia Coli. Los estudios más indicados para el diagnóstico fueron los cultivos de secreciones (56.48 por ciento) y el hemocultivo (19.4 por ciento). Conclusiones: El sexo masculino fue el más frecuente. Los intervalos de clases de edades 41-60 y más de 60 años fueron los grupos más infectados. Los gérmenes más aislados fueron el Estafilococus aureus, seguidos de la Klebseilla pneumoniae y la Echerichia coli. Los estudios más indicados para el diagnóstico fueron los cultivos de secreciones y el hemocultivo. El diagnóstico más frecuente fue el Hematoma Subdural Crónico. El tipo de sepsis fue la sepsis respiratoria y la flebitis(AU)


Introduction: In the history of Medicine, the study of infections dates from 1546 and extends to the present. The surgical approach of the Nervous System and the subsequent hospital stay makes this entity a constant threat for surgical success. Objective: To describe the behavior of infections in the Neurosurgery Service of Dr. MiguelEnríquez University Hospital. Material and Methods: A prospective study was conducted in 108 patients admitted to the Neurosurgery Service of Dr.Miguel Enríquez University Hospital who had infectious diseases from March 2013 to August 2015. The universe was composed of all patients older than 18 years of age, of both sexes, who were admitted to the service from different hospital areas: Emergency Room, Intermediate and Intensive Care Units, or directly admitted to the ward. Information was collected and included in a database, which compiled data such as number of the clinical history, age, sex, diagnosis, type of infection, indicated microbiological study, and the isolated germ. Tables of frequencies were built. Results: In this series, there was a predominance of the male sex (66.7 percent) in relation to the female sex (33.3 percent); the age group at highest risk was between the fourth and the sixth decades of life (75 percent); the most isolated germs were Staphylococcus aureus (18.5 percent), Klebsiella (7.4 percent), and Echerichia Coli. The most indicated studies for the diagnosis were the culture of secretions (56.48 percent), and blood cultures (19.4 percent). Conclusions: The male sex was the most frequent one. Age ranges from 41-60 and over 60 years of age were the most infected groups. The most isolated germs were Staphylococcus aureus, followed by Klebsiella pneumoniae and Echerichia coli. The most indicated studies for the diagnosis were the culture of secretions and blood cultures. The most frequent diagnosis was Chronic Subdural Hematoma. The type of sepsis was respiratory sepsis, and phlebitis(AU)

Background: Nosocomial infections (NI) are events associated with high impact on hospital costs and mortality. Aim: To evaluate from the health provider's perspective the costs and mortality attributable to NI. Methods: We selected a sample of patients with and without NI matched by age and diagnosis at admission. Costs were calculated and converted from Colombian pesos to US dollars using the average exchange rate of 2008. We evaluated the mortality rate in both groups. Results: We collected data on 187 patients with NI and 276 without NI. Median total hospitalization cost was US$ 6,329 (95% CI US$5,527-7,934) in NI patients, while in non-infected patients this median was US$1,207 (95% CI US$ 974-1,495). Mortality was higher in the NI group (31.6% versus 5.1%). Patients with NI had longer hospital stays (median 21 days, 95% CI 18-24 days) than non-infected patients (median 5 days, 95% CI 5-6 days). Mortality was also markedly higher in the NI group than in the non-infected group (31.6% versus 5.1%). Conclusion: NI are adverse and costly events related to patient attention that affect adversely the quality of attention.


Introducción: Las infecciones asociadas a la atención en salud (IAAS) están relacionadas con un incremento en los costos de hospitalización y un mayor riesgo de mortalidad. Objetivo: Establecer los costos y la mortalidad asociados a la presentación IAAS en una institución de cuarto nivel. Métodos: Se hizo una selección pareada de pacientes con IAAS y sin IAAS para calcular el costo por medio de costeo directo y emparejamiento. Los costos fueron calculados en pesos colombianos y convertidos a dólares estadounidenses según la tasa de cambio de 2008. Resultados: Se incluyeron 187 pacientes con IAAS y 276 pacientes sin IAAS. La tasa de IAAS fue de 1,8% La mediana del costo de hospitalización en los pacientes con IAAS fue US$ 6.329 (95% CI US$ 5.527-7.934) y en los no infectados de US$1,207 (95% CI US$ 974-1.495). Los pacientes con IAAS presentaron mayor tiempo de estancia hospitalaria, con una diferencia de 16 días respecto a los no infectados (21 días (IC 95% 18-24) vs 5 días (IC 95% 5-6)). Se encontró una mortalidad atribuible de 26,4%. Conclusiones: Las IAAS son eventos adversos a la atención, que se asocian con mayor mortalidad y generación de costos extra.

ABSTRACT

Objectifs : etudier la frequence des pathologies operees et relever les principaux germes retrouves au cours des infections de la plaie post-operatoire en chirurgie generale au CHR-Tsevie. Methodologie : etude retrospective sur les dossiers d'infections post-operatoires des patients operes dans le service de chirurgie generale au CHR - Tsevie du 1er Janvier 2012 au 31 Decembre 2013 (deux ans). Les parametres etudies etaient socio-demographiques; diagnostiques; le type d'intervention; le delai de survenue de l'infection; le traitement; l'evolution et la duree d'hospitalisation. Resultats : durant la periode d'etude 30 dossiers d'infections de la plaie post-operatoire ont ete colliges sur un total de 271 patients operes; soit 11%. Deux cent vingt neuf patients avaient ete operes en urgence soit 84;5% et 42 patients (15;5%) operes en chirurgie programmee. La sex ratio etait de 1;72. La tranche d'age de 19 a 30 ans constituait 40% des cas. Les patients etaient des cultivateurs (30%) et des eleves (23;3%). Les antecedents notes etaient : une hypertension (2cas); un diabete (1cas); une hypertension associe au diabete (1cas); un alcoolisme (30%) et un tabagisme (10%). L'infection post-operatoire a ete notee apres la prise en charge en urgence des affections comme la peritonite aigue generalisee (11cas/49 soit 22;4%) et l'appendicite aigue (6cas/34 soit 17;6%). L'antibio-prophylaxie a ete systematique chez tous les patients operes. Les germes retrouves etaient le Staphylococcus aureus (15 cas/50%); Escherichia coli (07 cas/23;3%) et le streptocoque (05 cas/16;6%). Les infections etaient notees entre 7 et 14 jours post-operatoire dans 70% des cas. La duree moyenne d'hospitalisation etait de 34 jours. Conclusion : l'infection de la plaie operatoire est frequente chez les patients operes en urgence. Les germes isoles a la culture sont souvent des bacteries cutanees. La prevention de l'infection du site operatoire necessite un respect rigoureux de regles d'asepsie

ABSTRACT

Objective: this study aimed to determine the incidence of nosocomial infections, the risk factors and the impact of these infections on mortality among patients undergoing to cardiac surgery. Methods: Retrospective cohort study of 2060 consecutive patients from 2006 to 2012 at the Santa Casa de Misericórdia de Marília. Results: 351 nosocomial infections were diagnosed (17%), 227 non-surgical infections and 124 surgical wound infections. Major infections were mediastinitis (2.0%), urinary tract infection (2.8%), pneumonia (2.3%), and bloodstream infection (1.7%). The in-hospital mortality was 6.4%. Independent variables associated with non-surgical infections were age > 60 years (OR 1.59, 95% CI 1.09 to 2.31), ICU stay > 2 days (OR 5, 49, 95% CI 2.98 to 10, 09), mechanical ventilation > 2 days (OR11, 93, 95% CI 6.1 to 23.08), use of urinary catheter > 3 days (OR 4.85 95% CI 2.95 -7.99). Non-surgical nosocomial infections were more frequent in patients with surgical wound infection (32.3% versus 7.2%, OR 6.1, 95% CI 4.03 to 9.24). Independent variables associated with mortality were age greater than 60 years (OR 2.0; 95% CI 1.4 to3.0), use of vasoactive drugs (OR 3.4, 95% CI 1.9 to 6, 0), insulin use (OR 1.8; 95% CI 1.2 to 2.8), surgical reintervention (OR 4.4; 95% CI 2.1 to 9.0) pneumonia (OR 4.3; 95% CI 2.1 to 8.9) and bloodstream infection (OR = 4.7, 95% CI 2.0 to 11.2). Conclusion: Non-surgical hospital infections are common in patients undergoing cardiac surgery; they increase the chance of surgical wound infection and mortality. .


Objetivo: O objetivo deste estudo foi determinar as taxas das infecções hospitalares, os fatores de risco associados e o impacto destas infecções na mortalidade dos pacientes submetidos à cirurgia cardíaca. Métodos: coorte retrospectivo que incluiu 2060 pacientes consecutivos, no período de 2006 a 2012 na Santa Casa de Misericórdia de Marília. Resultados: Foram diagnosticadas 351 infecções hospitalares (17%), sendo 227 infecções não cirúrgicas e 124 infecções cirúrgicas. As principais infecções foram: mediastinite (2,0%), infecção urinária (2,8%), pneumonia (2,3%), infecção da corrente sanguínea (1,7%). A mortalidade global intra-hospitalar foi de 6,4%. As variáveis independentes associadas às infecções não cirúrgicas foram: idade > 60 anos (OR 1,59; IC95%1,09-2,31), internação em UTI > 2 dias (OR5,49; IC95% 2,98-10,09), ventilação mecânica > 2 dias (OR11,93; IC95% 6,1 - 23,08), uso de sonda vesical >3 dias (OR 4,85 IC95% 2,95 -7,99). Infecções hospitalares não cirúrgicas foram mais frequentes em pacientes com infecção cirúrgica (32,3% versus 7,2%; OR 6,1; IC95% 4,03- 9,24). As variáveis independentes associadas a mortalidade foram: idade >60 anos (OR= 2,0 ; IC 95% 1,4-3,0), uso de droga vasopressora (OR 3,4; IC95% 1,9-6,0), uso de insulina (OR=1,8; IC 95% 1,2-2,8), reintervenção cirúrgica (OR=4,4 IC95% 2,1-9,0) pneumonia (OR=4,3 IC95% 2,1-8,9) e infecção da corrente sanguíneas (OR=4,7; IC95% 2,0-11,2). Conclusão: infecções hospitalares não cirúrgicas são frequentes pós cirurgia cardíaca, e aumentam a chance de infecção cirúrgica e a mortalidade. .

ABSTRACT

Objective: to study the predisposing factors that interfere with the emergence of infection in surgical wound after cesarean section. Method: It is an integrative review about the predisposing factors for infection of the surgical wound after cesarean section. It has as guiding question, which is the scientific production in the last five years about the predisposing factors for infection of the surgical wound after cesarean section? Results: We found five publications, which showed the main predisposing factors for infection: obesity, diabetes, low socioeconomic status, duration of labor and premature rupture of membranes. Conclusion: The subject is little explored in the scientific world and in the studies analyzed, concern over use of antibiotic therapy was superior to the conditions, which concern the pre, intra and postoperative great.


Objetivo: estudar os fatores predisponentes, que interferem no surgimento da infecção em ferida operatória pós-cesárea. Método: trata-se de uma revisão integrativa sobre os fatores predisponentes para infecção da ferida operatória pós-cesárea. Tem como questão norteadora qual a produção científica nos últimos cinco anos acerca dos fatores predisponentes para infecção da ferida operatória pós-cesárea? Resultados: encontramos cinco publicações, que apontaram como os principais fatores predisponentes para a infecção: a obesidade, diabetes, baixo status socioeconômicos, duração do trabalho de parto e ruptura prematura de membranas. Conclusão: O assunto é pouco explorado no meio científico e, nos estudos analisados, a preocupação com uso da antibioticoterapia foi superior às condições, que concernem ao pré, trans e pós-operatório ótimo.

Método: se trata de una revisión integrativa sobre los factores predisponentes para infección de la herida operatoria post-cesárea. Tiene como pregunta guiadora ¿cuál es la producción científica en los últimos cinco años acerca de los factores predisponentes para infección de la herida operatoria post-cesárea? Resultados: encontramos cinco publicaciones, que apuntaran como los principales factores predisponentes para la infección: la obesidad, diabetes, bajo status socioeconómicos, duración del trabajo de parto y ruptura prematura de membranas. Conclusión: El asunto es poco explorado en el medio científico y, en los estudios analizados, la preocupación con uso de la terapia antibiótico fue superior a las condiciones, que conciernen al pre, trans y post-operatorio óptimo.

OBJETIVO: Analisar a associação entre a obesidade materna e complicações infecciosas do puerpério em gestações de alto risco. MÉTODOS: Estudo prospectivo de ago/2009 a ago/2010, com os seguintes critérios de inclusão: puérperas até o 5º dia; idade >18 anos; gestação de alto risco; feto único e vivo no início do trabalho de parto; parto na instituição; peso materno aferido no dia do parto. O estado nutricional no final da gestação foi avaliado pelo índice de massa corporal (IMC), aplicando-se a curva de Atalah et al. (1997), e as pacientes foram classificadas em: baixo peso, adequado, sobrepeso e obesidade. As complicações do puerpério, investigadas durante o período de internação e 30 dias após a alta, foram: infecção e/ou secreção em ferida cirúrgica, infecção urinária, infecção puerperal, febre, hospitalização, uso de antibióticos e morbidade composta (pelo menos uma das complicações citadas). RESULTADOS: Foram incluídas 374 puérperas classificadas pelo IMC final em: baixo peso (n = 54, 14,4%); adequado (n = 126, 33,7%); sobrepeso (n = 105, 28,1%) e obesidade (n = 89, 23,8%). A obesidade materna apresentou associação significativa com as seguintes complicações do puerpério: infecção de ferida cirúrgica (16,7%, p = 0,042), infecção urinária (9,0%, p = 0,004), uso de antibiótico (12,3%, p < 0,001) e morbidade composta (25,6%, p = 0,016). Aplicando-se o modelo de regressão logística verificouse que a obesidade no final da gestação é variável independente na predição da morbidade composta (OR: 2,09; IC 95%: 1,15-3,80, p = 0,015). CONCLUSÃO: A obesidade materna no final da gravidez, em pacientes de alto risco, está associada de forma independente à ocorrência de complicações infecciosas no puerpério, demonstrando a necessidade de acompanhamento mais eficiente de ganho de peso materno nessas gestações.


OBJECTIVE: To analyze the association between maternal obesity and postnatal infectious complications in high-risk pregnancies. METHODS: Prospective study from August 2009 through August 2010 with the following inclusion criteria: women up to the 5th postpartum day; age > 18 years; high-risk pregnancy; singleton pregnancy with live fetus at labor onset; delivery at the institution; maternal weight measured on day of delivery. The nutritional status in late pregnancy was assessed by the body mass index (BMI), with the application of the Atalah et al. curve. Patients were graded as underweight, adequate weight, overweight, or obese. Postpartum complications investigated during the hospital stay and 30 days post-discharge were: surgical wound infection and/or secretion, urinary infection, postpartum infection, fever, hospitalization, antibiotic use, and composite morbidity (at least one of the complications mentioned). RESULTS: 374 puerperal women were included, graded according to the final BMI as: underweight (n = 54, 14.4%); adequate weight (n = 126, 33.7%); overweight (n = 105, 28.1%); and obese (n = 89, 23.8%). Maternal obesity was shown to have a significant association with the following postpartum complications: surgical wound infection (16.7%, p = 0.042), urinary infection (9.0%, p = 0.004), antibiotic use (12.3%, p < 0.001), and composite morbidity (25.6%, p = 0.016). By applying the logistic regression model, obesity in late pregnancy was found to be an independent variable regardless of the composite morbidity predicted (OR: 2.09; 95% CI: 1.15-3.80, p = 0.015). CONCLUSION: Maternal obesity during late pregnancy in high-risk patients is independently associated with postpartum infectious complications, which demonstrates the need for a closer follow-up of maternal weight gain in these pregnancies.

As infecções decorrentes de cirúrgicas limpas são importantes por sua morbidade, mortalidade e como marcador de qualidade da assistência. O interesse na identificação de fatores de risco para infecções em pacientes cirúrgicos tem aumentado entre profissionais de saúde. Estudos nesse sentido tornam-se cada vez mais necessários, uma vez que as infecções cirúrgicas aumentam o período de hospitalização, os custos da assistência e o sofrimento do paciente. Cirurgias de correção de fraturas de fêmur são frequentes entre os procedimentos cirúrgicos. O objetivo deste trabalho foi estudar os aspectos epidemiológicos da infecção de sítio cirúrgico em pacientes submetidos a cirurgias limpas para correção de fraturas de fêmur. Estudo tipo coorte histórica, identificou os fatores de risco para infecção de sítio cirúrgico (ISC) em pacientes submetidos a cirurgias limpas para correção de fraturas de fêmur em um hospital de grande porte de Belo Horizonte. A coleta dos dados foi feita nos registros dos prontuários eletrônicos no período de julho de 2007a julho de 2009. Foram coletados dados referentes às características dos pacientes, dos procedimentos cirúrgicos e das infecções cirúrgicas. Os fatores de risco para infecção foram identificados por meio de testes estatísticos de hipóteses bilaterais, considerando nível de significância de 5%. As variáveis contínuas foram avaliadas por teste t de student. As variáveis categóricas foram analisadas por meio de teste de qui-quadrado ou exato de Fisher, quando necessário. Para cada fator sob análise, foi obtida uma estimativa pontual e por intervalos de confiança de 95% para o risco relativo. Infections resulting from surgical clean is important for its morbidity, mortality, and as a marker of quality of care. The interest in identifying risk factors for infections insurgical patients has increased among health professionals. Studies accordingly become increasingly necessary, since the surgical infections increase the length of hospitalization, cost of care and suffering of the patient. Surgery for correction of femoral fractures are frequent among surgical procedures. The objective of this work was to study the epidemiology of surgical site infection in patients undergoing clean surgery to correct fractures fêmur. Estudo historical cohort, identified the risk factors for surgical site infection (SSI) in patients undergoing surgery clean to fix fractures ofthe femur in a large hospital in Belo Horizonte. Data collection was made in there cords of electronic medical records from July 2007 to July 2009. We collected data on patient characteristics, surgical procedures and surgical infections. Risk factors for infection were identified through statistical hypothesis testing bilateral level of significance of 5%. Continuous variables were evaluated by Student's t test. Categorical variables were analyzed using chi-square or Fisher's exact test when necessary. For each factor under study, we obtained a point estimate and confidence intervals of 95% for the relative risk. In the last stage of this work, we performed a multivariate analysis (logistic regression). The study included 432 patients who underwent correction clean fracture of the femur. The incidence rate of SSI was 4.9%and the risk factors identified were the presence of stroke (odds ratio - OR = 5.0) and hospitalization until surgery over four days (OR = 3,3). ABSTRACT

Objetivo: informar los resultados de la aplicación de la linfadenectomía estándar en el adenocarcinoma gástrico en el Hospital Clinicoquirúrgico Universitario Celia Sánchez Manduley, de Manzanillo, Granma. Métodos: se analizó la supervivencia postoperatoria para 5 años de 132 pacientes operados por adenocarcinoma gástrico, a los cuales se les realizó linfadenectomía de los 2 primeros niveles ganglionares. Se determinaron los resultados de parámetros epidemiológicos y se definió la supervivencia por el método de Kaplan-Mier. Resultados: las infecciones respiratorias, la anemia y la sepsis de la herida fueron las complicaciones más frecuentes, pero la oclusión intestinal mecánica tuvo significación con la mortalidad. La supervivencia fue superior a todos los reportes obtenidos en Cuba y mejor que algunos internacionales. Conclusiones: el empleo de la linfadenectomía estándar en el adenocarcinoma gástrico constituye un elemento esencial para establecer y mejorar el pronóstico en estos pacientes(AU)


Objective: to report the results of the application of the standard lymphadenectomy in cases of gastric adenocarcinoma admitted in the Celia Sánchez Manduley Clinical Surgical University Hospital of Manzanillo, Granma province. Methods: the 5-years postoperative survival of 132 patients operated on due to gastric adenocarcinoma that underwent lymphadenectomy of the two first ganglionic levels. The results of the epidemiological parameters were determined and the survival by Kaplan-Meier method was defined. Results: the respiratory infections, anemia and wound sepsis were the more frequent complications, but the mechanical intestinal occlusion had a significance related to mortality. Survival was higher to all reports obtained in Cuba and better than some internationals. Conclusions: the use of standard lymphadenectomy in cases of gastric adenocarcinoma is an essential element to establish and to improve the prognosis in these patients(AU)

Introducción: la génesis de las infecciones posquirúrgicas es multifactorial. Existen estudios internacionales que evidencian los diversos factores predictivos relacionados con la aparición de estas complicaciones, las que todavía afectan a un número considerable de pacientes intervenidos, todo lo cual justifica el interés en la realización de este trabajo. Objetivo: describir el comportamiento de algunos de los factores predictivos relacionados con la aparición de las infecciones posoperatorias en nuestro medio. Métodos: se realizó un estudio observacional, descriptivo y transversal de los pacientes ingresados y operados que presentaron infecciones posquirúrgicas en el servicio de cirugía general del Hospital Provincial Docente Saturnino Lora de Santiago de Cuba, durante el año 2008, según posibles factores predictivos seleccionados. Resultados: con relación al grado de contaminación, la tasa global de infección posoperatoria y la de heridas limpias estuvo en límites universalmente aceptados, aunque fueron más elevadas en las intervenciones urgentes, sucias y contaminadas para las localizadas en el sitio quirúrgico, así como en enfermos con estado físico preoperatorio según la American Society of Anaesthesiology (ASA) ASA II Y ASA III, con independencia de su estado nutricional y el tiempo quirúrgico en que se efectuaron las intervenciones. Conclusiones: las tasas de infecciones posquirúrgicas aumentaron en la medida en que fueron desfavorables las condiciones bajo las que se efectuaron las operaciones, y los factores predictivos seleccionados se relacionaron principalmente para las localizadas en el sitio quirúrgico, con el grado de contaminación, el tipo de intervención y el estado físico preoperatorio del paciente(AU)


Introduction: the genesis of the postsurgical infections is multifactor. The are many international studies evidencing the different prediction factors related to the appearance of these complications, which still involve a significant number of patients operated on, thus justifying the interest in the conduction of present paper. Objective: to describe the behavior the some of the prediction factors related to appearance of the postoperative infections in our practice. Methods: a cross-sectional, descriptive and observational study was conducted in admitted and operated on patients with postsurgical infections in the general surgery service of the Saturnino Lora Teaching Provincial Hospital over 2008, according to the selected prediction factors. Results: in relation to the contamination, the global rate of postoperative infection and that of the clean wounds are within the universally accepted limits, although are higher in the urgent, dirty and contaminated interventions for that located in the surgical site, as well as in patients with an ASA II and ASA III physical condition according the American Society of Anaesthesiology (ASA) independently of its nutritional status and the surgical time of interventions. Conclusions: the postsurgical infection rates increase as far as the conditions to operate on are unfavourable and the selected prediction factors are related mainly to that located in the surgical site, with de contamination level, the type of intervention and the physical preoperative status of patient(AU)

La contaminación bacteriana es el requisito indispensable para la aparición de las infecciones posoperatorias, y a pesar de los avances de las técnicas quirúrgicas, anestésicas y de la biotecnología, continúan asediando al paciente operado, aumentan su morbilidad y mortalidad, y tienen repercusiones socioeconómicas desfavorables. Se realizó un estudio observacional, descriptivo y transversal de 103 pacientes ingresados y operados de cirugías mayores que presentaron infecciones posoperatorias en el servicio de cirugía general del Hospital Provincial Docente Saturnino Lora, de Santiago de Cuba, en el año 2008, según variables seleccionadas. Objetivos: describir las principales características de las infecciones posquirúrgicas relacionadas con su localización, gérmenes aislados, antibioticoterapia utilizada y estadía hospitalaria, así como determinar la mortalidad por esta causa. Resultados: las localizaciones más frecuentes de las infecciones posoperatorias fueron: incisional superficial, respiratorias y de órgano/espacio para las intervenciones sucias y contaminadas. La media de estadía hospitalaria de la serie se elevó a 15,2 días. La mayoría de los gérmenes aislados fueron gram negativos, sensibles a los antibióticos de primera línea utilizados. Fallecieron 6 pacientes (5,8 por ciento): 5 por sepsis y uno por tromboembolismo pulmonar. El grado de contaminación bacteriana de la intervención es lo más significativo para la aparición de la infección posoperatoria, independiente de su localización. La política de utilización de antimicrobianos debe correlacionarse con la sensibilidad de los gérmenes aislados en el mapa bacteriológico de cada unidad asistencial. Las medidas preventivas deben comenzar en el preoperatorio, continuar a través de la intervención, y prolongarse en el posoperatorio, por las repercusiones socioeconómicas desfavorables de esta temible complicación posquirúrgica(AU)


Bacterial contamination is the essential requirement for appearance of postoperative infections and despite the advances of surgical, anesthetic techniques and of the biotechnology, still remains involving to the patient operated on, increasing its morbidity and mortality and unfavourable socioeconomic repercussions. A cross-sectional, descriptive and observational study was conducted in 103 patients admitted and operated on of major surgeries with postoperative infections in the general surgery service of the Saturnino Lora Teaching Provincial Hospital of Santiago de Cuba province over 2008, according to the selected variables. Objectives: to describe the major features of postsurgical infections related to its location, germs isolated, antibiotic-therapy used and hospital stay, as well as to determine the mortality due to this cause. The more frequent locations of postoperative infections were: superficial incision, respiratory and of organ/space for the dirty and contaminated interventions. The mean of the hospital stay of the series increased to 15,2 days. Most of the germs isolated were gram-negative, sensitive to first line antibiotics used. Six patients deceased (5.8 percent): 5 due to sepsis and one due to pulmonary thromboembolism . The level of bacterial contamination of the intervention is the more significant for the appearance of the postoperative infection, independent of its location. The politics for the antimicrobials use must to be correlated with the sensitivity of germs isolated in the bacteriological map of each assistance unit. The preventive measures must to begin in the preoperative period, over the intervention and to continue in the postoperative one due to the socioeconomic unfavourable repercussions of this fearsome postsurgical complication(AU)

Apesar dos avanços tecnológicos, pacientes submetidos ao ato anestésico-cirúrgico ainda têm complicações nas feridas operatórias. Essas feridas são classificadas como agudas e realizadas de modo a reduzir os riscos de complicações. Têm tendência à regressão espontânea e completa em um prazo esperado. No entanto, podem se tornar complexas e crônicas quando apresentam complicações no processo de cicatrização. Neste estudo tem-se como objetivos caracterizar os pacientes com ferida cirúrgica abdominal e pélvica tratados no setor de Estomaterapia de um serviço ambulatorial de um hospital em Belo Horizonte e comparar os fatores identificados nos pacientes com os citados na literatura como predisponentes às complicações na ferida cirúrgica. Trata-se de um estudo retrospectivo-descritivo, realizado no ambulatório com amostra de 22 pacientes portadores de ferida cirúrgica abdominal ou pélvica complexa. Houve predomínio de mulheres, idade variando de 22 a 79 anos, com sobrepeso ou obesidade. Todos os pacientes apresentaram deiscência na ferida cirúrgica com infecção associada em 63,6%, mais da metade das lesões apresentou área superior a 20 cm2 e a profundidade variou de 0,1 cm a 6,8 cm. A maioria recebeu alta com a ferida curada, em média, após três meses e dez dias de tratamento. Como descrito na literatura, os resultados obtidos confirmaram que o sobrepeso e a obesidade são fatores que contribuem para o desenvolvimento de infecção. Destaque-se, também, que a infecção e a deiscência foram os principais fatores que caracterizaram a ferida cirúrgica complexa.


Despite technological advances, patients submitted to surgical anesthesia procedures still suffer from surgical wound complications. These wounds are classified as acute and are made so as to reduce the risk of complications. They tend to retreat spontaneously and completely within an estimated time. However, they can become complex and chronic when there are complications in the healing process. The objectives of this study were to characterize patients with an abdominal and pelvic surgical wound treated in the Stomal Therapy Division in an ambulatory care department of a Belo Horizonte hospital and to compare the factors identified in patients predisposed to complications in the surgical wound with those reported in the medical literature. This is a retrospective, descriptive study taken at the ambulatory care with a sample of 22 patients presenting a complex abdominal or pelvic surgical wound. These were mainly women between 22and 79 years old, overweight or obese. The patients had surgical wound dehiscence associated with infection in 63.6%. More than a half of the lesions displayed an area greater than 20 cm2 and the wound depth ranged from 0.1 cm to 6.8cm. Most patients were discharged with the wound healed in about 3 months and 10 days of treatment. The results confirmed what is described in the medical literature, that is, overweight and obesity contribute to the developmentof infection. Infection and dehiscence were identified as the complex surgical wound main complication.


Aún a pesar de los avances tecnológicos pacientes sometidos a procedimientos anestésico-quirúrgicos suelen enfrentar complicaciones de heridas operatorias. Tales heridas están clasificadas como agudas y se realizan de modo a disminuirel riesgo de complicaciones. Tienen la tendencia a la regresión espontánea y completa dentro de un período esperado. Sin embargo, pueden transformarse en complejas y crónicas cuando hay complicaciones en el proceso de cicatrización. El objetivo del presente estudio fue de caracterizar a los pacientes con herida quirúrgica abdominal y pélvica tratados en el sector Estomaterapia de un servicio ambulatorial y comparar los factores identificados en los pacientes con aquéllos citados en la literatura como con predisposición a complicaciones de herida quirúrgica. Se trata de un estudio retrospectivo descriptivo realizado en ambulatorio con una muestra de 22 pacientes con heridas quirúrgico abdominal o pélvica complejas. Hubo predominio principalmente en mujeres con edades entre 22 y 79 años, con sobrepeso u obesidad. Todos los pacientes presentaron dehiscencia de herida quirúrgica asociada a infección en el 63,6%, más dela mitad de las lesiones mostró una superficie superior a los 20 cm2 con profundidad entre 0,1 cm y 6,8 cm. La mayoría fue dada de alta con la herida curada, en promedio, después de 3 meses y 10 días de tratamiento. Como descritos en la literatura, los resultados confirmaron que el sobrepeso y la obesidad son factores que contribuyen al desarrollo de la infección. También se observa que la infección y la dehiscencia son los principales factores que caracterizan la herida quirúrgica compleja.

Estudo de coorte histórica sobre infecções do sítio cirúrgico decorrentes das 3.543 cirurgias de pacientes ortopédicos constantes nos registros de banco de dados do Serviço de Controle de Infecção Hospitalar de um hospital geral, público e de grande porte de Minas Gerais. Os objetivos foram determinar a taxa de incidência de infecção de sítio cirúrgico e o tempo de manifestação da infecção, verificar a associação entre infecção e fatores de risco e identificar os microrganismos prevalentes. Encontrou-se a incidência de infecção de sítio cirúrgico de 1,8%. As variáveis potencial de contaminação da ferida cirúrgica, condições clínicas do paciente (ASA), duração da cirurgia e tipo de procedimento mostraram-se estatisticamente associadas à ISC. O tempo médio de manifestação das infecções após a cirurgia foi de 96 dias. O microrganismo mais frequente foi o Staphylococcus aureus.


This is a historic cohort study on surgical site infection occurred in 3,543 operations in orthopaedic patients. The surgical interventions were registered in the database of the Nosocomial Infection Control System of a Public General Hospital in Minas Gerais. Its objectives were to determine the incidence rate of surgery site infection, to verify the association between surgical site infection and some risk factors, to identify the prevalent microorganisms as well as the time for the infection's symptoms manifestation. The incidence rate of surgical site infection was 1.8%. The potential surgery wound infection, the patient's clinical condition (ASA), the surgery duration and the type of procedure were the variables statistically associated with SSI. After surgery the average time to the manifestation of infection was 96 days. The prevalent microorganism was Staphylococcus aureus.


Estudio tipo cohorte histórica sobre las infecciones de la herida quirúrgica como consecuencia de la cirugía ortopédica de 3543 pacientes incluidos en los registros de base de datos del Departamento de Control de Infecciones de un hospital general público de Belo Horizonte, Minas Gerais. Los objetivos fueron determinar la incidencia de la infección del sitio quirúrgico, determinar la asociación entre la infección y factores de riesgo, determinar el momento de aparición de la infección e identificar los microorganismos más prevalentes. Se ha encontrado una incidencia de infección del sitio quirúrgico de 1,8%. Las variables: potencial de contaminación de la herida quirúrgica, las condiciones clínicas del paciente (ASA), tiempo quirúrgico y tipo de procedimiento fueron estadísticamente asociados con la ISC. La infección se manifiesta en un plazo promedio de 96 días después de la cirugía. El microorganismo más frecuentes fue el Staphylococcus aureus.

Nontuberculous mycobacterial infections can cause destructive tenosynovitis of the hand. We report on and discuss the clinical course and distinctive radiologic findings of two patients with hand tenosynovitis secondary to M. marinum and intracellulare infection, which are different from those of the nontuberculous mycobacterial infections reported in the previous literature.

ABSTRACT

Sporadic reports on cardiac herniation are available in the literature; most of them had followed intrapericardial pneumonectomies for malignant pulmonary tumors. We present an uncommon event of heart herniation after a completion pneumonectomy indicated for chronic bronchiectasis. A 35-year-old male patient was operated for left completion pneumonectomy. A 6 cm Χ 4 cm area of adherent pericardium near the obtuse margin of heart was removed during surgery. During head-end elevation of the bed in postoperative intensive care unit, patient got accidentally tilted to the left side, which resulted in ventricular fibrillation. Chest cavity was re-opened for cardiopulmonary resuscitation. Left ventricle was found herniating through the pericardial deficiency into the left-thoracic cavity with the cardiac apex touching chest wall. During surgical re-exploration, the pericardial deficiency was closed with a synthetic Dacron patch. Hemodynamic condition remained stable in the immediate postoperative period. Patients had infection of the left thoracic cavity after 5 weeks, for which he was subjected to thoracoplasty and omentopexy. Prompt recognition with timely intervention is life saving from cardiac herniation. Strategy of closing the pericardial defect after pneumonectomy should be followed routinely, irrespective of the indication for pneumonectomy.

Este relato descreve a infecção pós-cesariana em puérpera de 20 anos que evoluiu com choque séptico após administração de hemotransfusão. Foi realizada laparatomia exploratória, histerectomia e colocação de Bolsa de Bogotá provisória até fechamento completo da parede abdominal. São revistos os fatores de risco para essa complicação e formas de prevenção.


Case report of postcesarean infection in 20 age puerpera that evoluted with septic shock and blood transfusion. A exploratory laparotomy followed by hysterectomy and use of Bogota bag temporary until the complete close of abdominal wall was made in the patient. In this article, the risk factors for this complication and the prevention forms are reviewed.

Introdução: A infecção de sítio cirúrgico é uma complicação grave da cirurgia vascular periférica. O recente aparecimento de estafilococos resistentes e agressivos gera uma nova preocupação com relação ao manejo e prevenção dessas infecções. O objetivo deste estudo é verificar a prevalência de resistência bacteriana estafilocócica, padrão de resistência bacteriana, a epidemiologia e possíveis fatores associados. Métodos: Pacientes submetidos a cirurgia arterial periférica no período de março de 2007 a março de 2008 que desenvolveram infecção da ferida operatória. Foram analisados 25 casos de pacientes portadores de infecção de ferida operatória causada por estafilococo. Os fatores estudados foram a presença de resistência bacteriana, idade, sexo, comorbidades, tempo de internação pré-operatória, profilaxia antimicrobiana, tipo de cirurgia, utilização de prótese, topografia da infecção, classificação da infecção. Resultados: Média de idade de 64,4 anos, 76% do sexo masculino. Prevalência geral de resistência bacteriana de 68%. O patógeno mais isolado foi o Staphylococcus aureus (64%), sendo 11 de 16 culturas (68,7%) resistentes à oxacilina. O padrão da resistência aos antimicrobianos foi: oxacilina 68%~ clindamicina, sulfametoxazol e eritromicina 64%. Todos os micro-organismos testados foram sensíveis à vancomicina. Conclusões: A resistência bacteriana é um achado frequente nas infecções de ferida operatória em cirurgia vascular periférica. O micro-organismo mais isolado é o Staphylococcus aureus, que demonstrou elevadas taxas de resistência aos antimicrobianos. A vancomicina permanece como a melhor alternativa para tratamento dessas infecções


Introduction: Surgical site infection is a serious complication of peripheral vascular surgery. The recent emergence of resistant and aggressive staphylococci raises a new concern as for the management and prevention of these infections. The aim of this study is to assess the prevalence of bacterial resistance to staphylococcal infection, pattern of bacterial resistance, epidemiology and associated factors. Methods: Patients who underwent peripheral arterial surgery in the period from March 2007 to March 2008 who developed wound infection. We analyzed 25 cases of patients with wound infection caused by staphylococci. The factors studied were the presence of bacterial resistance, age, sex, comorbidities, length of preoperative hospitalization, antimicrobial prophylaxis, type of surgery, use of prosthesis, infection topography, and classification of infection. Results: Average age was 64.4 years, 76% males. The overall prevalence of bacterial resistance was 68%. The most frequently isolated pathogen was Staphylococcus aureus (64%), and 11 of 16 cultures (68.7%) were resistant to oxacillin. The pattern of antimicrobial resistance was: 68% oxacillin, clindamycin, sulfamethoxazole and erythromycin 64%. All microorganisms tested were sensitive to vancomycin. Conclusions: Bacterial resistance is a common finding in surgical wound infections in peripheral vascular surgery. The most frequently isolated microorganism is Staphylococcus aureus, which showed high rates of antimicrobial resistance. Vancomycin remains the best alternative for treating these infections

CONTEXT AND OBJECTIVE: C-reactive protein (CRP) is commonly used as a marker for inflammatory states and for early identification of infection. This study aimed to investigate CRP as a marker for infection in patients with postoperative septic shock. DESIGN AND SETTING: Prospective, single-center study, developed in a surgical intensive care unit at Hospital das Clínicas, Faculdade de Medicina da Universidade de São Paulo. METHODS: This study evaluated 54 patients in the postoperative period, of whom 29 had septic shock (SS group) and 25 had systemic inflammatory response syndrome (SIRS group). All of the patients were monitored over a seven-day period using the Sequential Organ Failure Assessment (SOFA) score and daily CRP and lactate measurements. RESULTS: The daily CRP measurements did not differ between the groups. There was no correlation between CRP and lactate levels and the SOFA score in the groups. We observed that the plasma CRP concentrations were high in almost all of the patients. The patients presented an inflammatory state postoperatively in response to surgical aggression. This could explain the elevated CRP measurements, regardless of whether the patient was infected or not. CONCLUSIONS: This study did not show any correlation between CRP and infection among patients with SIRS and septic shock during the early postoperative period.


CONTEXTO E OBJETIVO: A proteína C reativa (PCR) é muito usada como marcador de estados inflamatórios e na identificação precoce de infecção. Este estudo teve como proposta investigar a PCR como marcadora de infecção em pacientes em choque séptico no período pós-operatório. TIPO DE ESTUDO E LOCAL: Estudo prospectivo, monocêntrico, desenvolvido numa unidade de terapia intensiva pós-operatória do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. MÉTODOS: Foram avaliados 54 pacientes no pós-operatório, sendo 29 deles com choque séptico (grupo SS) e 25 com síndrome da resposta inflamatória sistêmica (grupo SI). Todos os pacientes foram acompanhados durante sete dias pelo escore SOFA (Sequential Organ Failure Assessment) e com dosagens diárias de PCR e lactato. RESULTADOS: As dosagens de PCR não diferiram entre os grupos. Não foi observada correlação entre dosagem de PCR e lactato ou escore SOFA nos grupos estudados. Observamos que as concentrações plasmáticas de PCR estavam elevadas em quase todos os pacientes avaliados. Os pacientes no pós-operatório apresentam estado inflamatório em resposta à agressão cirúrgica, sendo este fato capaz de explicar as dosagens de PCR elevadas, independentemente de o paciente estar ou não infectado. CONCLUSÕES: Este estudo não evidenciou correlação entre PCR e infecção nos pacientes com síndrome da resposta inflamatória sistêmica e choque séptico no período pós-operatório precoce.
